pN1a indicates that cancer cells have metastasized (spread) to at least 1 to 3 lymph nodes. At least one must be larger than 2mm. pN1b is a sign that cancer cells are present in lymph nodes located behind the breastbone (the inner mammary nodes). These areas can be detected with a sentinel biopsy, but they are difficult to feel.
